REFLECTIONS at PLUS: A 33 bed geriatric dementia unit designed for individuals with Alzheimer's or other related dementia.  Medicaid or Private Pay accepted.
BRIDGES - EAST: A 25 bed skilled care unit designed for individuals requiring a maximum level of patient care.  Medicare, Medicaid or Private Pay accepted.
BRIDGES - WEST: A 25 bed routine care unit designed for individuals requiring moderate to maximum patient care.  Medicaid or Private Pay accepted.
INDEPENDENCE WAY: A routine care unit designed for individuals requiring minimum to moderate patient care.  Medicaid or Private Pay accepted.
THE EVERGREEN BEHAVIORAL UNIT:
The Evergreen Behavioral Unit offers a secured unit for persons experiencing behavioral and emotional challenges. Emotional challenges coupled with a medical diagnosis may result in a compromised quality of life. More information on Evergreen Behavioral Unit
