Behavioral Program Admission Guidelines

Before admitting a resident to the Evergreen Behavioral Health Program, some issues must be considered. An initial pre-admission screening must be completed for residents who have mental illness or mental retardation. The Admission Team will identify and consider several factors prior to accepting the resident.

Program Features Include:
  • Consulting Psychiatrist
  • Family Involvement and Education
  • Licensed Social Worker for individual and group therapy
  • Daily Activities
  • Staff trained and supported in behavioral care management
  • Medication evaluation and management by psychiatrist
  • Occupational and physical therapy offered
  • Recreational therapy programs
  • Individualized behavioral treatment plans

    Admission Criteria:

    • All potential admissions will be evaluated by Clinical Manager or Assessment R.N.
    • Individuals must be at least twenty-one years old for admission to unit
    • The resident must have a primary medical diagnosis with a secondary psychiatric diagnosis of such severity that long-term placement is needed
    • The resident's physical and medical condition must allow active participation in activities
    • Pre-admission assessment includes direct resident assessment and history taking
    • Persons who may not be appropriate for admission include: dementia, bedridden, isolation residents, and anyone who is actively experiencing suicidal ideation

    Consideration of Inclusion Factors:

    • Primary Medical Diagnosis With Secondary
      Mental Health Diagnosis
    • Psychiatric, Nuerological Or Other Diagnosis
    • Guardianship Status
    • Degree Of Compliance With Care
    • History Of Suicidality
    • Presence Of History Of The Following:
      -Self-motivation Or Self-injurious Behavior
      -Aggresive Behavior
      -Inappropriate Sexual Behavior
      -Elopement
    • Reasons For Discharge From Previous Settings

    Exclusive Criteria:

    • Drug Abuse Requiring Detoxification
    • History Of Repeated Or Intentional Arson
    • Homicidal History Or Behavior
    • Violent Sexual Criminal Behavior
    • Current Active Suicidality
    • Repetitive Successful Elopement
